Deluxe Plan

*Coinsurance out-of-pocket maximums from $2,000 – $10,000
Advantage Plan
Reduces premiums by offering two out-of-pocket maximums; one applying to medical services and supplies, and the second to surgical services and inpatient confinement.
*$1,000 – $25,000 deductible options
*80% coinsurance
*$3,000 coinsurance out-of-pocket maximum for medical supplies & services; $6,000 coinsurance out-of-pocket maximum for inpatient confinement and surgery
Premier Plan
Offers a daily deductible; covered charges that exceed daily deductible amount on any give day are paid at 100% when visiting in-network providers.

High-Deductible Health Plan (HDHP)
